There are many useful files here for download. If you’re planning to see me for consultation, I recommend that you definitely download the office policies and the initial visit info form. This is not essential, so if you hate filling out forms I can get the information when we get together.

I suggest that you locate any assessment tools that pertain to the problems you’ve been having, fill them out, and bring them with you to your first appointment. You can also email me a scanned copy so that we have it when you visit with me. Some of the files may appear as duplicates, as sometimes I am not satisfied with a particular scanned version. I add to this list from time to time. As far as I know, everything here is in the public domain, so please let me know if that is not the case.
